Workers’ Compensation Visits at The Urgent Care: What You Need to Know

At The Urgent Care, we’re here to support injured workers by providing fast, professional care through approved workers’ compensation claims. To help streamline the process and avoid delays, please review the following steps and requirements before your visit.

What You’ll Need to Bring:

All workers’ compensation patients must have their employeer submit a signed Workers’ Compensation Form. This form is required at check-in and must be completed before being seen.

If a form is not submitted, we require that an approval letter or email from the insurance adjuster be sent to us before the patient’s visit. This must be submitted to Fax@TheUrgentCare.com and should include:

1. Insurance Information

  • Insurance company name
  • Adjuster’s name, phone number, and email

2. Employer Information

  • Company name
  • Address and phone number
  • Employer contact

3. Patient Information

  • Full name & date of birth
  • Date of injury
  • Description of Injury
  • Location of Injury
  • Claim number (if submitted and SSN if not submitted)

4. Treatment Authorization

  • The approval must specifically state that the patient is allowed to be treated by a mid-level provider (NP/PA). We cannot guarantee that a physician will be available for treatment.

For Employers & HR Managers:

To help your employee be seen promptly, make sure the approval and all necessary documents are sent in and submitted before the visit.
