If you are applying for a green card or adjusting your immigration status, you may be required to complete an immigration medical examination performed by a designated civil surgeon. At The Urgent Care, our civil surgeon can help eligible patients complete the medical examination required by U.S. Citizenship and Immigration Services (USCIS). To ensure a smooth and efficient visit, please bring the following items to your immigration physical appointment:
- Photo ID (passport, visa, driver’s license, employment-authorization card, or any other government-issued ID)
- Vaccine records (if available)
- Vaccination records must include the dates that the vaccines were administered
- If you do not have complete vaccination records, our team can help explain what additional documentation or vaccinations may be needed
- Any medical records that may be relevant to your examination
- Any immigration-related notifications or letters received from USCIS, as they need to be included in the sealed envelope.

The process typically requires two clinic visits:
- First Visit: Complete paperwork, medical exam, and lab tests.
- Follow-Up Visit: Review results, sign documents, and pick up the sealed envelope.
The sealed envelope is ready in 5–7 days, provided all lab results are normal and vaccine records are complete.
Delays may occur if additional tests (e.g., chest X-ray) or medical treatment are required due to abnormal lab results.
The Immigration Medical Exam is required for filing Form I-485 (Application for Lawful Permanent Residency or Adjustment of Status). Once you have your “Green Card ,” you can become a citizen in 3 – 5 years, depending on your case.
