This form is required at check-in and must be completed before being seen.<\/p>\n\n\n\n<p>If a form is not submitted, we require that <strong>an approval letter or email from the <a href=\"https:\/\/www.insurance.com\/expert-opinion\/what-does-an-insurance-adjuster-do\/\" title=\"insurance adjuster\">insurance adjuster<\/a><\/strong> be sent to us <strong>before the patient\u2019s visit<\/strong>. This must be submitted to <strong><a>Fax@TheUrgentCare.com<\/a><\/strong> and should include:<\/p>\n\n\n\n<p><strong>1. Insurance Information<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Insurance company name<\/li>\n\n\n\n<li>Adjuster\u2019s name, phone number, and email<\/li>\n<\/ul>\n\n\n\n<p><strong>2. Employer Information<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Company name<\/li>\n\n\n\n<li>Address and phone number<\/li>\n\n\n\n<li>Employer contact<\/li>\n<\/ul>\n\n\n\n<p><strong>3. Patient Information<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Full name &amp; date of birth<\/li>\n\n\n\n<li>Date of injury<\/li>\n\n\n\n<li>Description of Injury<\/li>\n\n\n\n<li>Location of Injury<\/li>\n\n\n\n<li>Claim number (<em>if<\/em> <em>submitted and SSN if not submitted)<\/em><\/li>\n<\/ul>\n\n\n\n<p><strong>4. Treatment Authorization<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>The approval must specifically state that the patient is allowed to be treated by a mid-level provider (NP\/PA). <em>We cannot guarantee that a physician will be available for treatment.<\/em><\/li>\n<\/ul>\n\n\n\n<h4 class=\"wp-block-heading\">For Employers &amp; HR Managers:<\/h4>\n\n\n\n<p>To help your employee be seen promptly, make sure the approval and all necessary documents are sent in and submitted <strong>before the visit<\/strong>. <\/p>\n\n\n\n<p>Questions?
